(DM to LD): Thanks will add your initiative there. And you actually forgot to add your initiative modifier of +5 so you actually got good initiative roll for this round: 20. Remember that you can make a Perception Check on your turn only as it is a minor combat action. So right now we have not yet decided the order of the turns, Make Skill Check later on your turn. Everyone else can make it too on their respective turns.

(DM to Terence): You tried to hit Rat 3 AC of 15 but got only 13 to attack roll so you failed to hit him. You can't use piercing strike again this combat since it's a encounter power.

As the dire rat that had gotten close to her fell, Kiyamera took a quick look around to see what was going on. On her left side, Brandis was in the middle of a swing when he hit one of the dire rats besides him. At the same time Balasar missed with his magic once again. Kiyamera could not see the farmer or the tiefling. Then she thought she saw something in the pool of dirty water..

Now let's see if Rat 4 falls on the pool or not as you attempt to use diabolic grasp to move him:
Rat 4 Saving Throw: 1d20 = [18]. As he got more than 10 he is saved and has not fallen on the poll.
(Brandis attack on rat 6): Brandis missed Rat 6 with his lightning lure attack and provoked OAs from Rat 1 and 2.
