To be honest. I did not like the sanctuary faction from H6. So I wouldn't mind if they didn't come back.

Expect them to be in the near future. Most people did like them. And as one of the core 9 Factions, they will pop their snake-like heads round the door sooner rather than later.

First I need to express my point of view regarding Sanctuary. To me, this faction is a good addition to current universe for two reasons that I tie together : a new faction (such as Fortress in V) yet having, at least to me, nostalgia elements -Nagas.
I also appreciate very much the lack of history background this faction has, avoiding alterations such as Minotaurs going from their cool mythological background (living in labyrinth, willing to devour all who enters) to being Frankensteins enslaved by Dark Elf O_o.
A lack of history tends to trigger the player's imagination, which I see as a good element regarding lore in general.

Considering possibility of seeing Sanctuary again through eventual expansion(s), below are my suggestions.

Since Medusae are, in some symbolic way, going back to Dungeon, their replacement with Lizardmen here seems to not only be a nice aspect towards nostalgia, but also as a way to prevent a possible overkill of it, featuring somehow “good” vs “evil” Medusae. Sanctuary also being a reptilian faction, I cannot see any reason why it shouldn’t happen, and when you see Lizardmen were shooters in Heroes III, it just looks to me like the spot is made for them.
I also enjoyed much the Japanese mythology's (water spirits) inclusion into the Indian one (Nagas), and therefore would favor a replacement for Mizu-Kami, because of alleged similarity with Yuki-Onna, both visually and conceptually. I see a spot in elite that could benefit a more diverse creature.

I would disagree with the vision of OP : I believe the whole feel of Sanctuary in Heroes VI has potential to offer something more mystical to the game’s universe, and as such wouldn't make a change in that aspect, though is to note, that unlike the OP, I like Sanctuary from the start.

Except that Medusa were never a part of Sanctuary.
Medusa are simply Naga who worship a forbidden aspect of their Goddess Shalassa, the destructive aspect known as the Wrecker. Any Naga who does so is a Medusa. Any and all Medusa are banished. Not just the female ones with the Snakehair. Many female Naga are born with Snakes as hair, having such does not a Medusa make.

So no, Medusas will not and need not be replaced. Naga with Snakehair will always be a part of Sanctuary, unless Naga are kicked out all together.

Underlining my opinion for them to move out, Medusae vs Renegade Medusae seems visually a bit redundant. I obviously wouldn't like Naga to be kicked out all together.

True but those weren't Nagas.

I loved the h6 sactuary faction as a whole. The design, the theme and atmosphere where all perfect. They just needed more naga based units

That is a fair point. However, since we’re not getting back Swamp Fortress anytime soon I wouldn’t mind it being temporary. As for the Shintoistic composure, design can take care of it.

I think you might misunderstand "composure" in my sentence.
I intended it in the synonym of equanimity: 'the state of being calm, stable and composed, especially under stress'. This is rather incompatible with savagery. That is not something design can take care of.

I didn’t even notice the typo, and certainly wouldn't permit myself to correct it as I personally struggle with expressing myself through proper English.
I agreed with you that design alone isn’t sufficient for the equanimity, implying that it requires creative writing to the lore in order to  have Lizardmen match the town. I also think it could be done in a way that doesn’t tie the unit to Sanctuary, so that it can freely move to an hypothetical Swamp Fortress if it ever happens in a further title of the franchise.
