None of above. The most important is the minimap, in 99% of cases it shows if quality. If I like the minimap, I download then look in editor if design is of quality. Then I play if I like what I see, in all cases the one who  pays attention to design will pay same attention to game play and vice-versa.

I agree, it is probable that I have already selected the size and difficulty when I look for a new map in Maps4Heroes but for sure I pick the ones that look nicest. Then there is of course the hear about, I didn't found TDS or To Kill for Power because of their look but because I've read great comments about them here. I pay very little attention to the ratings cause I tend not to like the ones with 10s (very difficult and with a big-brothers's sense of control I truly hate). Generally, I only like freeruns and most of the times these aren't the ones that get 10s.

See the comments first, they might giveaway something in use for you. Like if the map has a lot of flaws. Some people post images as well. Second consider using a MapEditor to load the map and browse for 10 secs the enviroment, that way you can see the "way" it is made. Teleports/towns/obstacle etc. For example I dont play a map which is FULL with events and obstacles.

As well have in mind that the new maps let's say for HotA can be double edged sword, but you can never go wrong with RoE map which is with high rating and positive reviews. Also if you want to buff it a bit in your editor before playing it would take less than 30 minutes to do so. Here you go...
Cheers!
