I deliberately avoid telling people my birthday, because whenever people are extra nice to me, I feel ashamed because deep inside I think I'm worthless and deserve to be ignored. This is something I have had my entire life. When I was very young there was a particular Christmas where I cried because I had been given "too many presents". From then on my family had a tradition of spreading out the gift opening over three days so as not to overwhelm.
Besides, birthdays are overrated. In the words of Gregory House "Ah, my birthday. Normally I'd put on a festive hat and celebrate the fact that the Earth has circled the Sun one more time; I really didn't think it was going to make it this year, but darn it if it wasn't the little planet that could all over again."

Presents are only to be taken symbolic IMO, and you shouldn't really get overwhelmed with them. Also I do agree that those that really care about you should be nice with you the whole year and not just one day when it's your birthday.
