How to make a map from a picture?

I've seen some maps with fantastic details which resemble pictures. My question is, is there some program which converts images in terrain in the map editor? Or is everything made by hand.
If there is such a thing, are there any requirmnets for the picture used?

You have option to import bitmap in map editor. Just install the newest version of editor and that option will appear on minimap in editor.
